Welcome to the Tolleson Union High School District! We are pleased to share the various employee benefits available to you. Please take the time to carefully review your benefits packet given to you at the time of hire. There is a waiting period for benefits. Benefits become effective on the first of the month following the 30-calendar day waiting period. If you choose not to enroll for any of the insurance coverages within 30 days of your initial hire date, you will have to wait to enroll during the next open enrollment period *. The only exception to this rule is if you and/or your family experience a qualifying event such as loss in health coverage with another employer, marriage, birth and/or adoption and the new enrollment is completed within 30 days of the qualifying event. (Employees who work 30+ hours weekly, or a minimum of .5 FTE for teachers, are eligible for medical/dental benefits. Employees who work a minimum of 15 hours are eligible for the Basic Flexible Spending Plan benefit.)

Benefits Fair

In the Spring of each year, a Benefits Fair is held at each school site. Benefits representatives are available at this event to assist you with questions and share valuable information. The best part is the freebies given to you just for stopping by their booth. You may even have a chance to win some great prizes. Don’t miss out on this exciting event! Information will be E-mailed and posted at your school.

Open Enrollment *

Open enrollment allows eligible employees to make changes to their benefits (i.e., add/drop dependents, change plans, enroll for new coverage). Open enrollment is for a specified period only, usually during the month of May. Employees who miss open enrollment will have to wait until the next calendar year’s open enrollment period.

This plan allows you to choose a physician in-network only, there are no out-of-network benefits. $400/$1200 Deductible for employee/family. Inpatient Hospitalization, Outpatient X-Ray and Surgery covered 80% only. $30 Physician co-pay and prescriptions range from $15 - $50 (+Cost Diff). See Benefit Summary for other benefits outlined.

ASBAIT PPO, A-15 Plan– Option #2
This plan allows you to choose any physician in or out of network. This is a PPO which gives you the flexibility of seeing a physician without a referral. In-network co-pays are $15.00 for PCP or Specialist, $100 Inpatient Hospitalization, $65/$35 ER/UC. There is NO deductible and prescriptions range from $15 - $50 (+Cost Diff). Eye Med has thousands of provider locations, including optometrists, ophthalmologists, opticians, and Lens Crafters locations throughout the country. Eye exams have a $20.00 co-pay. Other benefits include, $100.00 allowance for frames; 80% of balance covered if over $100.00, Lenses are free, $115.00 allowance for Contacts, plus 15% discount of balance, 15% discount off Lasik surgery.

Life Benefit
One times annual salary, rounded to the next higher $1000, subject to a minimum of $15,000 and a maximum of $100,000. This benefit is paid for by the district.

AD&D (Accidental Death & Dismemberment)
One times annual salary, rounded to the next higher $1000, subject to a minimum of $15,000 and a maximum of $100,000.

Example: If your death is a result of an accident and your annual salary is $30,000, your beneficiary will receive $30,000 for the life insurance and $30,000 for the accidental death, for a total of $60,000.

BASIC Flex Plan - IRS SECTION 125 PLAN
Plans for Medical Cost Reimbursement and Dependent/Day Care

For employees who work 15 or more hours p/wk

BASIC Flex is a flexible spending plan, established as part of Section 125 of the Internal Revenue Code. As a participant in flexible spending, you will receive a mySourceCard MasterCard Debit Card. You assign a certain dollar amount to be transferred from your gross pay for 21 pays. The maximum dollar amount is $5000 per plan year. These dollars are set aside in a special account through payroll deduction and expenses are reimbursable as they occur. You save by not paying federal, social security and state taxes on the amount you set aside. In effect, you create a tax shelter for these dollars. The result is that you have more spendable income by participating in a flexible spending plan. Be sure to base your estimate on KNOWN expenses since leftover money cannot be reimbursed.

ADDITIONAL-TERM LIFE AND SHORT TERM DISABILITY INSURANCE

Voluntary benefits may be purchased at your discretion for employees who work 20 or more hours per week. Enrolling in additional Term Life and Short Term Disability outside of open enrollment or new hire is subject to medical screening, paid for by the employee. Please note there is NO district contribution for these coverages and the cost will be paid by the employee through payroll deductions. Appropriate forms must be completed.

Term Life
Term Life Insurance is payable to the designated beneficiary upon the death of the insured. Coverage is provided for the time period that you are eligible and premium is paid. There is no cash value associated with this product. Rates and more information are available upon request in the District Human Resources Office.

Short Term Disability
Short Term Disability Insurance is available at various rates; 8-week or 13-week benefit. Rates and more information are available upon request in the District Human Resources Office.

Arizona State Retirement System (ASRS) - Long Term Disability

Eligible employees have long-term disability benefits through their membership in the Arizona State Retirement System (ASRS). Eligibility for long-term disability benefits begins after six (6) months of disability, subject to approval. VPA, Inc., administers the ASRS long-term disability benefit program.

ASRS Contributions
The current ASRS employee contribution for eligible employees is .5% for LTD and 9.1% for retirement for a total of 9.6% contribution.
