Athletic Clearance Procedures

Students must complete the following eligibility requirements before being allowed to try-out or practice in interscholastic competition:

  • The Emergency Care and Parental Consent form must be completely filled out along with the Annual Pre-participation Physical Evaluation form with parent/athlete signatures on ALL designated areas.


  • An Annual Preparticipation Physical Examination form is required and must be successfully completed and signed by a licensed M.D., D.O., N.P., or P.A. (Physicals taken on or after March 1 of this year are good for the following school year.)


  • Proof of medical insurance must be provided. A copy must be kept on file in the athletic office and be updated every school year. It is the school district’s policy that all students participating in interscholastic activities must have insurance and that the school cannot pay any medical cost from injury to a student.


  • A Birth Certificate must be brought into the athletic office the first time a student wishes to participate in athletics. The birth certificate is copied and returned immediately. It will not have to be brought in again in subsequent years.


  • An Annual Athletic Fee of $50.00 is due at the beginning of the season. This is payable at the student bookstore.


  • Parent(s) and student must view the Informed Consent Video prior to beginning any sport, after which, an Athletic Statement of Awareness form must also be completed.
  • A Domicile form must also be completed each year.


  • Fill out and sign the Equipment Check-Out form.


  • Bring completed forms to the athletic secretary at least one week prior to try-outs or practice to receive a clearance slip. Do not give forms to the coach or trainer.
